Abstract
Project Aim: To improve the triage process to enable clinicians to reach timely & safe decisions about eligibility by June 2026. Some referrals to South Warwickshire CLDT cannot be clearly identified as having a learning disability. The triage process can vary from case to case, and from clinician to clinician. In some cases, it can be months before a definitive decision can be made, as to whether referrals meet CLDT criteria. This project aimed to improve the triage process, so clinicians felt more able to reach timely and safe decisions about eligibility. Project Impact: New process implemented to aid quicker decision making. Pre- allocated initial assessment sessions scheduled in diaries. Reasonable adjustment clinics. Reduction in the use of extended triage tools and a move straight to initial assessments with clinicians. The project did not progress through a PDSA cycle, nor was it able to fully measure its success, however it has informed a wider LD transformation project with elements of this new process being incorporated into the new Access Hub process.
