    Abstract
In this article, we explore the potential role of patient-reported outcome measures as a bridge between health and social care, clarifying current applications and future opportunities. We explore how cross-cutting patient-reported outcome measures could support better integration of health and social care services, better health outcomes, and enable increased independence and choice for people, as consumers of social care.
