To establish a safe and effective process of transfer of patients between all professionals within Planned Therapy Services by the 26 August 2022. The team identified a lack of a defined auditable process for the effective and efficient management of transferring patients between staff groups. They worked together to streamline the transfer process to ensure that it was clear and concise and to ensure that workload was distributed fairly. Project Impact: A safe and effective process of transfer of patients was established. The percentage of staff who agreed or strongly agreed that workload was distributed fairly increased from 40% to 70% and the percentage of staff who agreed or strongly agreed that the process for transferring patients between staff groups was clear and concise increased from 10% to 70%.
